Captive Set Free is a telehealth medication management service with a Psychiatric Advanced Practice Nurse to provide a comprehensive psychiatric evaluation for children, adolescents, and adults in Illinois in order to prescribe the best medication possible to treat mental health symptoms.

How It Works

Schedule an initial psychiatric evaluation in our HIPPA compliant client portal IntakeQ.
You will receive an email verifying the appointment along with electronic consent forms.
If billing insurance, Headway will send you email directly related to payment details, such as co-pay and session cost ahead of time.
Telehealth appointment, we will send you a link to join the appointment at a scheduled time. The appointment will be by video using a device such as a smartphone, tablet, or computer.
In-office setting available in Peoria, Illinois the last Thursday of each month.
You will meet with a provider to discuss your mental health concerns and symptoms.
If medication is prescribed, it will be sent electronically to a pharmacy of your choice.

About Sherri

I am an MSN, RN, APRN-FPA (Advanced Practice Registered Nurse – Full Practice Authority) as a board-certified psychiatric mental health nurse practitioner. I would consider it a privilege if you would allow me to walk with you during the struggles you may face on your mental health journey.
The mental strength in each client is made stronger through a connection with God and others. My ability to connect well with the heart of my clients has afforded me many opportunities to watch my clients overcome their mental health struggles.
I have over 20 years of experience as a Registered Nurse obtaining my Master’s Degree in Psychiatric Mental Health in 2017. I am licensed to prescribe medication for depression, anxiety, bipolar, ADHD and other mood symptoms. Your mental health in safely prescribing medication for mood is my utmost priority. I have open flexible scheduling to help facilitate clients’ mental health concerns and would consider it a privilege to work with you.
